Please follow these instructions to reserve your appointment :

1. Click the button below to book your appointment through the Hawkesbury Family Health Organization.

2. Click on the “Patient” icon.

3. If your newborn is not a Hawkesbury Family Health Organization patient, click “No”. If he is, click “Yes”.

4. Select “Dr. Bercier – Ottawa” as the location. Read the instructions and click “Okay”.

5. Select “Dr. Bercier – Ottawa/Gatineau – Circumcision” as the type of appointment.

6. Select the date and time of the appointment. (The system will automatically direct you to the next available date.)

7. Fill out all requested sections.
For “NOTIFICATION METHOD” select “Email”. We do not communicate via text message.
For ”IDENTIFICATION”, choose the province of your newborn’s health card and enter your newborn’s health card number. If he does not have a health card number yet, enter the mother’s health card number.

8. Review the information, accept the condition, and submit the request.

9. Click on the “Start Pre-visit Qnaires” icon.

10. Answer all questions in the questionnaire as precise as possible. Once complete, review the information and make note of the appointment date and time in your calendar.

At this point, the appointment will be temporarily reserved. Prior to confirming the appointment, we will need to receive your payment. You will receive an email within a few days with information on how to complete thIS step.
