4. Select “Dr. Bercier – Hawkesbury” as the location. Read the instructions and click “Okay”.

5. Select “Dr. Bercier – Hawkesbury – Vasectomy Reversal Consultation” as the type of appointment.

6. Select the date and time of your appointment. (The system will automatically direct you to the next available date.)

7. Fill out all requested sections.
For “NOTIFICATION METHOD” select “Email”. We do not communicate via text message.
For ”IDENTIFICATION”, choose the province of your health card and enter your health card number.

8. Review the information, accept the condition, and submit your request.

9. Click on the “Start Pre-visit Qnaires” icon.

10. Answer all questions in the questionnaire as precisely as possible. Once complete, review the information and make note of your appointment date and time in your calendar.

At this point, the appointment will be temporarily reserved. Prior to confirming the appointment, we will need to receive your payment and/or a referral from a medical doctor or nurse practitioner. You will receive an email within a few days with information on how to complete these two steps.
