What Is the ShangRing™?

The ShangRing™ is a one-time-use, sterile device specifically designed for adult male circumcision. It comprises two plastic rings: an inner ring lined with soft silicone and an outer ring that works together to secure the foreskin during the procedure. This design reduces bleeding and ensures proper wound healing. The ShangRing™ circumcision is performed using the “no-flip” technique, a method that minimizes complications, pain, and downtime compared to traditional circumcision methods. Available in various sizes, the ShangRing™ ensures a custom fit for each patient, contributing to the procedure’s comfort and success.

Although the risks of complications are minimal, they are not nonexistent. There is a 4 to 5% probability of developing edema, a 1 to 2% risk of infection, and a 1 to 2% risk of wound dehiscence (wound reopening). The risk of bleeding is less than 1%. Although urinary retention has not been reported in studies, it remains a rare possibility. Research indicates that dissatisfaction with the aesthetic results of ShangRing™ circumcision is exceptionally rare, with rates reported between less than 1% and 3%.

The ShangRing™ is not suitable for everyone. It is contraindicated for individuals with urinary tract infections, active genital infections (such as genital warts), bleeding disorders, or anatomical abnormalities of the penis, including hypospadias. Additionally, if the foreskin is too tight due to a condition called phimosis, a small incision may be necessary to facilitate the device’s placement.

Preparing for Your Procedure

Before your circumcision, determining the correct size of the ShangRing™ is crucial for a successful outcome. This process involves measuring the penis’s circumference while in a relaxed state, just below the glans. Accurate sizing helps ensure the device fits securely and functions as intended.

To manage pain, a topical anesthetic ointment specially designed by Dr. Bercier in collaboration with a local pharmacy, containing various anesthetics, is generously applied to the skin of the penis, the foreskin, and the glans. If necessary, a syringe is used to apply the cream inside the foreskin. After the cream is applied, the area is covered and left for 60 minutes to allow the anesthetic to take full effect. Once the anesthetic has numbed the area, any remaining ointment is carefully wiped off, and the penis is disinfected with an antiseptic solution.

The ShangRing™ Procedure

The ShangRing™ procedure begins with preparing the foreskin. If it adheres to the glans, the doctor will carefully separate it. For patients with tight foreskins, a small incision may be made to facilitate the device’s placement. The inner ring of the ShangRing™ is then inserted under the foreskin at a 45-degree angle and positioned close to the frenulum, a band of tissue on the underside of the penis. The outer ring is then secured over the inner ring, trapping the foreskin between the two.

After the rings are locked in place, the excess foreskin is removed with scissors, and the wound is cleaned with antiseptic solution. The design of the ShangRing™ minimizes bleeding and ensures a clean, safe procedure. Before completing the operation, your doctor will confirm the device is securely positioned and functioning correctly.

Recovery and Aftercare

Recovery after a circumcision with the ShangRing™ is straightforward but requires careful attention to ensure proper healing. Rest at home for the first 24 to 48 hours following the procedure to allow your body to adjust to the device. It is important not to attempt to adjust or remove the ShangRing™ yourself, as this could lead to complications. Wear snug-fitting underwear to support the penis in an upward position, reducing the risk of swelling for up to two weeks after the ring falls off. Sleep on your side to avoid discomfort during the night.

The device usually falls off on its own between 7 and 21 days after the procedure. While the area must be kept clean and dry for up to two weeks after the ring falls off, you may shower the day after the procedure. Avoid rubbing the area; simply let soapy water flow over it, then rinse with clear water. Gently dry the penis and the device afterward. You may also use a hairdryer on a low temperature to accomplish this. If the wound remains moist, the risk of complications increases. Avoid swimming or bathing for up to two weeks after the device has fallen off.

For the first 7 days, avoid physical activities that could displace the ring. We also recommend postponing intense physical activities that may cause excessive sweating for up to two weeks after the ring has fallen off. If sweating occurs, ensure the area is dried to keep it clean and dry.

If an erection causes discomfort, apply the cold pack we provided to the top of your scrotum and the base of your penis, over your athletic supporter or underwear.

Sexual activity and masturbation are strictly prohibited while the ShangRing™ is in place and for up to two weeks after it has fallen off, or until the wound is completely healed, which can take up to 7 weeks. For your first sexual activity, we recommend wearing a condom until the sensation returns to normal. Always use a condom to prevent sexually transmitted infections (STIs) with multiple partners or as a method of contraception.

For mild pain, you may take the medications (acetaminophen and naproxen) we prescribed as needed. Also, apply the antibiotic powder provided by Dr. Bercier 1 to 3 times a day to the circumcision site once the area is clean and dry, until the product is finished. Do not apply any other creams, ointments, or products to the area.

Once the ShangRing™ has completely fallen off, you may use the dressings provided to you. Change them daily or more often if they become wet, until you have used all the dressings.
